WHAT IS RENUVION?

We mention Renuvion under the Cellulite reduction heading and it certainly reduces cellulite via skin tightening.  Up until 2022, I used Profound for skin tightening and although I was happy with Profound, Renuvion blows Profound out of the water.  Profound was labor intensive.  It took hours and there was about a week of downtime with bruising and swelling.  What is a week if the results are amazing!?  Well with Profound the results were just OK.  Renuvion also carries about a week to 10 days of downtime, BUT TOTALLY WORTH IT.   Renuvion only takes about 20 minutes once you have local anesthetic infiltrated. 

HOW IT WORKS

Just to compare, Profound, a radio-frequency (RF) device, emits probes just below the skin, to tighten. The Profound RF heats the area to 67 degrees Celsius for 3 seconds. Over a decade of research determined these parameters to be the best for skin tightening without harming the overlying skin.  Renuvion, however, heats the deep dermis to 85 degrees Celcius.  This should cause sever burns, but the Helium plasma cools in less than a second.  Thus, Renuvion achieves maximun tightening without harm to the overlying skin.

Renuvion is designed to deliver results in a single treatment, not a series.  This is not a procedure for an aesthetician or medical assistant to perform.  Tumescent fluid must be injected carefully and great care taken to place the Renuvion probe throughout the treatment area.  It is imperative to use the Renuvion probe at specific spacing and different levels. You can expect some swelling and perhaps bruising.   You will need to wear a compression garment for a while after the procedure.  This helps the newly remodeled skin to re-adhere tightly and securely.  

EXPECTED RESULTS

Renuvion is not a surgical body lift, however, you will get results to 50% to 80% of what a surgical procedure will yield.  The trade off is no surgery, no incision and its resulting scar, less recovery but still incredible results.  Dr. Treece and Southern Aesthetics will go over the details in detail with you whe you schedule a consultation at the Metairie office.

Abdominoplasty is a major surgical procedure in which excess abdominal skin and fat are removed, and the muscles of the abdominal wall are tightened. This procedure is particularly helpful to those who have abdominal excess due to weight loss or a tendency to have gained weight in this area over the years despite diet and exercise. Often pregnancies can stretch or “pull apart” the abdominal wall muscles to the point where no amount of diet and exercise will return their abdomen to its former shape. The results are phenomenal, but not without downtime of usually 6 weeks.  We will discuss the procedure along with the risks and complications at your consultation. six

But do you even need surgery? We also offer CoolSculpting, Profound, and Velashape procedures that may adequately address your needs. The point is we have many solutions that will all be discussed when we meet you.

Please note that photos are taken 6 weeks after surgery. This will give you an idea of your early scarring. Scars generally fade over the next 6 to 12 months.

Brachioplasty

Brachioplasty is a procedure performed to remove the “waggle” under or on the back of the arms that can occur due to ageing or after weight loss. This can also be a result of heredity. After every attempt to exercise away this area results with “no results”, there is something I can do. Options include cutting away the excess skin and fatty tissues either with a scar down the back of the arm or limiting the scar to the armpit. Liposuction is usually performed at the same time.

 

The scar down the back of the arm can be seen in short sleeves, but that’s the trade off. As one patient put it, “Yes, there’s a scar down the back of my arm. Life is full of scars and before my arm lift I couldn’t even squeeze these wings into a turtleneck sweater; you could still see the big bulges.” The armpit scar is much less noticeable but determining what you need depends on where we are starting and what you hope to accomplish. You can return to normal work duties soon but laborious work and strenuous exercise will need to wait six weeks after this procedure.

Pregnancy, weight gain, and aging can all cause unsightly broken leg vessels. Basically, the veins have been under high pressure causing them to dilate and become visible. They can be tiny (spider veins) or large and ropey, like the size of a finger (true varicose veins). True varicose veins may require testing for valve patency and may require surgical intervention. Most medium to tiny vessels can be treated with sclerotherapy or laser. Once the high pressure condition has developed in the veins, unfortunately the problem tends to be ongoing, but we can curtail this problem and help prevent future occurrences.

I like to start with sclerotherapy. This involves injection of concentrated saline into the visible veins. Immediately, you see the veins go away, but then compression must be maintained for a period of several weeks to allow the veins to completely regress. I find that sclerotherapy is less painful than laser, and it heals and resolves more quickly as well. We will discuss the post procedure protocol at your consult, but know that compression is essential so this procedure is best done in cooler weather.  Although you must compress, you CAN get back to your daily activities and exercise the very next day.

I reserve our vascular laser for resistant veins. The laser “sees” the color of the veins and has an overall “blast effect”. This works very well to destroy the vein, but can leave behind hemosiderin staining, a discoloration that can last for extended periods and may never completely clear.

The treatment takes about 15 minutes, and to be forewarned, it does nip a bit, but it’s not nearly as painful as seeing the broken vessels glaring back in the mirror. The best plan of action will be formulated at your consultation appointment. Usually at least two sessions are required and perhaps more, depending on the severity.

Cellulite Reduction Treatments

Cellulite occurs because there are tissue connections between the deep dermis and the fascia that cover the muscle beneath.  Add to this extra subcutaneous fat packing between these tissue connections (think tufted sofa) and skin sagging with age, or both, and the result is the dreaded appearance of cellulite. The thinner the skin, the more apparent this is.  Men don’t typically have cellulite because of thicker skin and less subcutaneous fat. 

What’s the solution? Decrease the fat, tighten the skin, and build the muscle.  Hyperdilute Radiesse, Renuvion, Emsculpt Neo and the Toledo Techniques all accomplish these goals.  It is complicated to choose which is best for you.  Dr. Treece is accomplished at Hyperdilute Radiesse, Renuvion, Emsculpt Neo and the Toledo Technique.  In fact, Dr. Treece had personally experienced these techniques so she can tell you exactly what to expect.    At Dr. Treece and Southern Aesthetics in Metairie you will experience a comprehensive consultation to decide what choices will serve you best.

This injectable melts the fibrous connections that form the pockets where fat accumulates.  When the fibrous connections are gone, the fat can smooth out evenly.  Multiple treatments are ususally needed 

HYPERDILUTE RADIESSE

Radiesse is an FDA approved filler that has been around for some 20 years.  It is different from all the other fillers in that it is composed of calcium hydroxylapetite.  All other fillers are composed of hyaluronic acid.  The composition of Radiesse makes it perfect to flow into and fill out the indentations of cellulite.  The “hypderdilute” part means Dr. Treece thins out the Radiesse so that it flows more easily into the cellulite defects.  

How much Radiesse you need will be determined by the extent of the cellulite and the area treated.  Typically, two sessions are needed at least 6 weeks apart.  As a filler and like all fillers, Radiesse will eventually fade away, BUT, research shows that as Radiesse goes away, 35% NEW collagen is left behind.  This means the results are very long lasting and may be essentially permanent provided you maintain a healthy diet and good muscle tone.  A lot depends on your skin and lifestyle:  sun and time cause thinning skin allowing cellulite to become more visible. 

Fat Reduction Non-surgical: CoolSculpt, Emsculpt Neo, and Kybella

I purchased m first CoolSculpt machine at the beginning of 2011. It seemed too good to be true (and what did your Mama tell you?). We demoed the machine and, for the first time ever, we got the same amazing results touted in the brochure.   Actually my Mama has been happy to have CoolSculpt prover her wrong and has taken full advantage of the Coolsculpt fat freezing miracles.

 

CoolSculpt is  like a “clamp” that sucks your fatty area up very tightly. As the freezing begins, it does pinch a bit, but only for a few minutes, and then you just stay still for an hour. After we “unhook” you, we massage the area and you just go on with your life. The area will be sore for a while but you can exercise and there is no girdle to wear like with liposuction. The fat cells that are treated undergo a type of cell death called apoptosis.  The fat is then  absorbed and eliminated by the body naturally. It is NOT a replacement for liposuction as it only removes 20-30% of the fat in the area and it can take up to 3 months to see the full results. CoolSculpting can be repeated, removing another 20-30%. If you don’t want downtime or surgery, Coolsculpting  is an amazing option for you.  Of importance, CoolSculpting is just a “tool” in Dr. Treece’s big box of fat removal tools.  CoolSculpting is best performed by a physician that is familiar with and has practiced liposuction, abdominoplasty, kybella, and Emsculpt Neo.  Otherwise, you cannot be assured that CoolSculpt is the best solution for you.  Also, the placement of the CoolSculpt head is crucial to achieve optimal fat removal without creating a defect in surrounding tissues.  Ultimately, CoolSculpt may not be the best option for your fat removal goals.  Dr. Treece is an expert at all forms for fat loss, and together you will decide your perfect solution.  

 

Hand Rejuvenation

How many times have we seen an aging actress and think she looks great? Then we see the hands! Hands often give away our real age, or worse, make us appear older than we really are. They should not be neglected! We address them with two approaches:
 
  1. Skin discoloration is treated with our lasers and/or peels in combination with home care.

  2. The veiny, boney appearance is disguised with filler. This is also quick with minimal downtime. How much you will need depends upon the severity of the problem and what you wish to achieve. Filler can be added over time to achieve the final results, which are long lasting.

Scar Revision Treatment

First we need to determine the CAUSE of your scar to determine the appropriate solution. Reasons for scarring are multiple: poor initial treatment, poor subsequent treatment, hypertrophic results, atrophic results, genetics, etc. Then we need to know what TYPE of scar it is: keloid, hypertrophic, atrophic, pigmented, depressed, etc. Once we pinpoint the cause and type of the scar, we can better devise a treatment. Treatments solutions are: excision of the scar or effectively “starting over”, a series of microneedling or laser treatments, steroid injections, or radiation therapy which can be the case for resistant keloids. Sometimes the solution is a combination of several therapies. Consultation is the only way to devise a plan.
 
Liposuction is one of the most common cosmetic procedures performed in the United States today, and under current guidelines and in experienced hands, it is also very safe.
Liposuction can be performed to slim the hips, thighs, tummy, ankles, calves, knees, arms, back, love handles and chin. Patients of any age can undergo liposuction, but those who have greater skin elasticity will achieve the best results.
Often a patient will say, “I don’t want Liposuction, just the Laser Lipo.” Laser Lipo does involve suctioning. With both procedures, tiny incisions are made. We use the Tumescent technique to help decrease bruising and improve pain post surgery. After tumescence, the procedures differ:
  • Traditional Lipo: We use the Microaire instrument to aid in breaking apart the fat, making it more easily suctioned, and thereby cause less trauma and pain in the surrounding tissues.
  • Laser Lipo:  We use a laser to partially liquify the fat, making it more easily suctioned and thereby causing less trauma and pain in the surrounding tissues.
Basically, there are two ways to skin a cat! BOTH techniques then require the fat be suctioned out of the body. Which do I feel is better? It totally depends on the individual.

Vaginal Rejuvenation

Vaginal Rejuvenation encompasses a whole slew of questions and problems. Although commonly thought of as the “vagina,” the external parts that that we commonly consult for are: the labia majora, labia minora and mons pubis. These are not part of the “vagina”. The vagina is the area extending internally from these external parts, within the uterus up to the cervix. Complicating this area further is the urethra (the tube you urinate through), so urinary problems can be associated with this area. The surgical procedures involved would address aesthetic, functional or both problems: Labiaplasty, Vaginoplasty (posterior colporrhaphy), Bladder Lift/Suspension, and Liposuction/Profound to the Mons Pubis.

LET’S BREAK THIS DOWN

I have done surgical labiaplasty for some 24 years throughout my career, but this does not address “internal ” issues. Labiaplasty involves surgically reducing the excess tissue of the labia minora and/or majora. Until you have seen what I have seen, you may think this would not be a problem! BUT this tissue can be so excessive as to “pop out” of shorts not to mention bathing suits and it can even become irritated, painful, and infected from intercourse or activities such as jogging. Conversely, maybe you just don’t like how yours “looks” either from childbirth, or you may have been born with excess. To be clear, Labiaplasty can address both these aesthetic and functional problems. The procedure involves removal of the excess skin from the area. This may sound quite painful but surprisingly, it is not, and heals quite quickly. It does involve refraining from certain activity until you are completely healed.
Vaginoplasty, or posterior colporraphy, is much more extensive and is used to address the INTERNAL anatomy that has become extensively stretched due to time or childbirth. In this procedure, a bowel cleansing is required before surgery. During surgery, the internal vagina and the underlying muscles are excised in a pie shaped wedge and then sewn back together in layers. Severe bladder issues often complicate this issue (as an enlarged vagina will likely stretch the urethra) and need to be addressed surgically as well. Often a plastic surgeon and a urologist or urogynecologist with work in concert to accomplish all the repair work necessary as a bladder lift or suspension may be required. I find when I consult for vaginal rejuvenation most of my patients are concerned more with appearance and minor functional problems, so I leave this extensive procedure to the urogynecological experts. As my practice evolved, I did not do a lot of these procedures, so ultimately I don’t do them at all. If you need this extensive work, make sure you see an expert who primarily does this procedure.
